When calling into a specific personal extension at the Decorah office, please add a number "6" in front of the old three-digit extension; therefore, all extensions are now four digits and begin with a "6."
Address:
Ortivus has used two mailing addresses in the past: 2324 Sweet Parkway Road and P.O. Box 276. Effectively immediately, Ortivus requests that mail sent through the postal service be directed to the P.O. Box 276 address, as the 2324 Sweet Parkway Road address will no longer be active for postal service deliveries after November 1, 2008. UPS, FED EX and other shipments can still be directed to the 2324 Sweet Parkway Road address, which will remain the physical address of the Decorah Ortivus office.
